Health Plans
PATIENT FINANCIAL POLICY
In order to reduce confusion and misunderstanding between our patients and the practice, we have adopted the following financial policy. If you have any questions about the policy, please discuss them with our staff. We are dedicated to providing the best possible care and service to you. We regard your complete understanding of your financial responsibilities as an essential element of your care and treatment.
- Your insurance policy is a contract between you and your insurance company. As a courtesy, we will file your insurance claim for you.
- We have made prior arrangements with several health plans to accept assignment of benefits. We will bill those plans for which we have an agreement and will only require you to pay the authorized copayment at the time of service. These companies are listed on the back of this page.
- If you have insurance coverage with a plan that we do not have a prior agreement with, we will prepare and send the claim for you. If payment is not received from the insurance company within 45 days, we will look to you for payment.
- For all services we provide to you in the hospital, we will bill your health plan. Any balance due is your responsibility and is due upon receipt of a statement from our office.
- For all services rendered to a minor patient, we will look to the parent or guardian with custody for payment.
- If you have no insurance and wish to set up a payment schedule, we will be glad to do so.
- For patients not having insurance coverage, we do have a discount policy. A discount of 15% will be extended to patients if the payment is received within 15 days of an elective surgery and within 20 days of an emergent surgery. If the patient status is other than a one-day surgery status, the payment due date will be based upon the patients discharge from the hospital. Office visit fees are not included in the discount offer.
